Service Coordinator - @ 4040 Market St., Philadelphia, PA 19104

Work Schedule:  8 am to 4 pm

Position Location:   4040 Market Street, Philadelphia, PA

Primary Function:  This position is responsible for the development and coordination of interagency, child and family linkages in the community.  The Service Coordinator position will ensure that all early intervention services for children and their families are coordinated and comprehensive.  Responsibilities include services for children, writing IEP's, and meeting with families.

Education:   Bachelor's Degree with PA Certification in Special Education.

Immediate Supervisor:  Director of Service Coordination

Contacts:   Elwyn personnel, providers of Early Intervention, Parents/Guardians, School District of Philadelphia, ChildLink, Mental Retardation Services, advocates, community agencies, Community Behavioral Health.

Experience:  Two (2) years of experience or more in early intervention or two years of experience as a case manager/service coordinator.  Bilingual skills a plus!
